3) Education details
4) Passport size attested photographs :two
5) Electricity Bill(Residence)
6) For Shop/ Business/Professions:Registration Certification from MCD
7) Industrial unit in non-confirming area: noc of HPC & MCL
8) Affidavit in prescribed format, duly attested by the NOTARY PUBLIC.
